Missed/Cancel or Reschedule appointment policy.

  • We have a strict policy regarding missed/cancel or rescheduling appointments. You must call us at least 48 hours in advanced if you can not make it to your appointments. If you fail to call us within this time frame, you will be charged ($30.00 for a hygiene appointment) and ($50.00 for a treatment appointment) this will be per child. This fee is your responsibility, and it will NOT be covered by any insurance.
  • After 3 missed appointments the entire family will be dismissed from our practice.
  • If you are running late all appointments have a 15-minute grace period, except for the last appointments of the day (11:30am and 4:30/5pm) will only get 10- minute grace period) PLEASE CALL OUR OFFICE IF YOU ARE TARDY.

First appointment

  • For your first appointment, you must fill out the paperwork on our website or come 30 minutes earlier to fill it out in our office.
  • All patients must come with a parent or (legal guardian with court documentation) for their first appointment.
  • Please make sure to bring your insurance card and any x-rays from your previous dentist.

Treatment appointment

  • For all treatment appointments patients must come with their parents or (legal guardian with court documentations)

**The only patients allowed to come by themselves are those 18 and older. **

Copays /balances due at time of visit

  • All insurances have copays that patients are responsible to pay. We will try our best to call parents the day before any appointments to let them know of any balances or copays due. However, parents are responsible for calling their insurance and finding out their copays.
  • If you cannot make any payments at your appointment, we will have to reschedule your appointment.
  • If your child is over 18 and will be coming by themselves, they must bring a form of payment to their appointment.

Mask if sick

  • We don’t have a mask policy, but we do appreciate that if you are sick to please wear a face mask if you want to sit in the waiting room or if you would like to come in with your child. If you prefer to not wear a face mask when sick, please wait in the car.
  • IF ANY CHILD IS SICK, PLEASE CALL OUR OFFICE AHEAD OF YOUR APPOINTMENT TO MAKE SURE THEY CAN STILL COME.

Food and Drinks

  • Our office doesn’t allow food and drinks to be brought in. Please keep these items in the car.

Electronics

  • We don’t allow pictures to be taken in the room or in the bay. We do have a little photobooth patients are welcome to take pictures with. This is all due to HIPPA privacy policy.
  • If you would like to come in with them to the room or bay, all phones and computers MUST REMAIN IN YOUR BAG OR POCKET.

Updates

  • Please make sure to keep our office updated with (email addresses, mailing addresses and phone numbers). This will be our only way to contact you.
  • Please update our office with any changes in your insurance (make sure your insurance is in network with us PPO Plans)
  • Please update our medical staff when you are being signed in regarding any medical changes.
